Workscope Submission:
- Assessment Installation Form
- Homeowner Agreement or Rental Property Agreement
- House Diagram
- Appliance Exchange Agreement (On Appliance Enrollment otherwise on EmPower+ Enrollment)
- Energy Usage Waiver – if no bills are available
- NEC Calculation Worksheet – Required when proposing an upgrade to a panel box
- Field Change Order Form – Required for all workscope changes after approval
- SHPO Form – if needed
- Customer Attestation (Heat Pump Acknowledgement Form) – Only required for enrollments with ASHP/GSHP measures (previously Heat Pump Demonstration Projects)
Final Project Submission:
- Clean and Tune Checklist – Required when Clean & Tune measure proposed
- Contractor Attestation (Certificate of Completion-LMI Heat Pump Projects) – Only required for enrollments with ASHP/GSHP measures (previously Heat Pump Demonstration Projects)
- Pre-Install Measure Report – (except with enrollment type is Audit Install) – Generated in NYHEP after the workscope has been approved
- Post Install Report (Certificate of Completion) – (except with enrollment type is Audit Install) – Generated in NYHEP after project has been completed
